That's a good question- For me, it helped very much to ease my symptoms. The prostate produces about 25% of the seminal fluid. (My reasoning was that shrinking it should create less semen.)
Plus, it's relatively harmless to shrink the prostate by using medicines. I think it's a worthwhile thing to try.  Saw Palmetto is an herb used in Europe and America to reduce the prostate, and it is available without a prescription. I use it currently together with a prescription medication also intended to shrink the prostate. I think it keeps my prostate pretty small!

I also use saw palmetto extract as a part of my health regiment to try to keep my prostate size at bay.  An enlarged prostate can start to interfere with the bladder.  The first sign of this is a strong sense of urgency when you have to go pee.  Like all of a sudden it hits you and you have to go now!  This is because the prostate is pushing on the bladder which messes up its ability to sense how 'full' you are.

So you did take finasteride before surgery?, how much did it lessen your sympthoms? 20 percent? Dont you have any sides from finasteride? How many do you take it? 5mg?

Hi Starsky, Yes, I started  Flomax (Tamsulosin) in 2007- a year before my surgery. I recently switched to a more powerful one- Avodart 0.5mg (Dutasteride).
Flomax did improve the symptoms both physically and mentally. I don't remember a quantitative amount (percentage) of improvement. But it was significant... and it didn't have any noticeable side effects for me, so I was glad to stay on it.
I recently switched to the Avodart, because I wanted the most powerful one. The Avodart works very well and got me to being 100% POIS free.

Animus, do you think if you had removed just one testicle, the one which caused you the most grief, if would have been enough to solve the POIS?

This is exactly what the urologist told me would happen. But I wanted to see if it would work anyway. But apparently this is what happens when you remove just one testicle. I reluctantly removed the second testicle. I had to pay for the operation twice because of that.

I decided that "dry ejaculation was the solution. And it seems to have worked 100%. The testicles =contribute about 10% I think of semen volume. The other glands are also really responsible for a lot too. Prostate, Seminal vesicles. The testicles are the most symbolic of glands.  i hope to get prosthetic implants some time soon.

"the gains" you initially got, you mean it initially helped the POIS? but then went back to normal?

I "feel" POIS is less of a semen problem and more a testicular problem (or sorts), just my observations

what were the state of yours prior to the operations?
what made you choose one over the other? was one sore to touch? was the problem one smaller?

my problem one is smaller than the bigger other one, you see
did you have a scan, im going to get scan, maybe we should all compare scans!

That's my impression as well.

BTW, a good part of the size can be contributed by the epididymis. Sometimes (often?) there are volumnous veins weaved in among the epididymis, which can give quite a voulme to the "testicle".

I personally have always had the elididymis on the left, with more volume than the testicle itself.

WITHOUT RESEARCH THERE WILL BE NO CURE!
Sessions 5 to 9 days, mostly Flu-like, joints, digestion problems, light cognitive.
Niacin has changed my lif though, now 1 day MAX.
Somewhere in this interaction with Niacin is the answer!

one of my testicles gives me alot problems, its sore,

it is significantly smaller than the other.

even my urologist noticed, but had no treatment

Since your blood tests in immunology were all normal (as mine almost), how could you have thought it was a semen allergy/ immunitary reaction?
Do you think it could be a new type of reaction that the science doesn't really know instead of auto immune real reaction?
One member had some semen tests and it was not positive allergic, but no negative either... ???

Brain fog 90%  + tired all the time ,sport intolerance, fast heartbeat, colon inflammation

I understand what you are saying and your concerns about the painful testicle. The state of my testicles before the operations was that they had become abnormally enlarged, and were also painful, tender, and sore at times.

There was not any deformity other than size and pain which the urologist could point to.
I would definitely suggest you have the testicle looked at.  Especially if one is sore. There is a condition where the sperm cord can get twisted and wrapped the wrong way- but this is extremely painful and would be an emergency. However, I think you should try to find some answers as to what's going on. This could be pointing to something.

It's true that one testicle is often slightly smaller than the other.

I did have partial gains from the removal of one, which only lasted a few months.

that's an interesting question. I myself did not come to the conclusion that it was a semen allergy/ immune reaction.  (although I don't think it's a bad theory). My primary theory was of an inability to comfortably regenerate the semen after ejaculation- which was causing the POIS symptoms.  

Hm. I think it's interesting that some members can have the allergic reaction, and others not. There seems to be a bit of a disconnect there although they must be working on it.

hi friends,
i am off pois since the last 2 days,but i have got pain in my left testicle.it had happened many times before and i am not sure how many days it will last,but i have noticed it happens if i have multi orgasms maybe in a couple of days..if i have an O after a gap of maybe 10-15 days or more then this pain does not come.that means the orgasm should be full load..even a pain which exists,does go away then..but if i have these frequent ones like i had in this last week,then my left testis pains...

Since the semen is still at the same place, and we have a little POIS... then something doesn't work in the Jigsaw !

Yes, it's true that arousal can cause milder POIS symptoms.
There is some fluid secretion due to arousal- I think it is called pre-cum. It's a thick, clear, viscous fluid. I think the purpose of it is to neutralize the vagina so sperm can survive, and a few other things...
There is definitely some fluid produced and secreted before full ejaculation. So arousal is setting in motion secretions and possibly triggering semen regeneration to begin as well... However, it could also be triggering an allergic reaction.

So you're saying if you have multiple orgasms the testicle pain is more severe. I can understand that. I also had that problem too. It feels tender, painful and very sensitive. There are definitely a few of us with this problem.
It could be that the testicles are undergoing a kind of trauma due to multiple ejaculations.
I checked online and found some brief therapies for it, which I've also tried. I found the support and elevation helps.

Trauma: After careful evaluation, if no serious underlying testicular injury have been identified, most cases of testicular trauma can be managed and treated at home. Treatment consists of the following measures:

    pain medications, including anti-inflammatory agents;

    scrotal support and elevation;

    ice packs; and

    rest.

More severe cases of testicular trauma involve testicular rupture, blunt trauma with an associated hematocele (a collection of blood) and penetrating traumatic injuries requiring surgical intervention.
