I also responded to a comment on why I believe POIS is heavily undiagnosed:

It's heavily undiagnosed for a few reasons:
1) Many who realize they feel worst following ejaculation do so through "semen retention" or "NoFap" challenges i.e. 10 day NoFap/semen retention abstinence challenges. If they feel better or experience "benefit", they then attribute the success of these experiences to the "power" of semen retention. They dive into the exciting literature that confirms their experiences on the 'benefits of semen retention' which is thrown around heavily in these online echo chambers i.e. 'The Coiled Serpent', 'The Practice of Brahmacharya', and even higher quality resources like 'The Way of the Superior Man' mention the importance of ejaculating to preserve consciousness. The literature also addresses reasons for feeling bad after ejaculation i.e. depletion of precious 'life force energy'. By online echo chambers, I am mostly referring to the r/semenretention subreddit.
2) I hypothesise there are many men out there who aren't even aware their ejaculation is causing negative symptoms. Most dudes who discover "semen retention" or POIS did so after a period of complete ignorance of their existing condition. To diagnose this, one needs to consciously stop ejaculating for at least 10 days, which isn't really a natural thing for men to do.
3) Talking to a doctor or family about feeling worst after ejaculating can be embarrassing. The topic of ejaculation is taboo and uncomfortable to talk about.
4) Many doctors are completely unaware of POIS. It's not very well known, which presents its own set of unique problems
