Legendary_animal found some relief with a combination of Diclofenac (50 mg) and Paracetamol (325 mg), while Charles_b found relief with aged garlic extract, so these could be worth trying in this context.

My symptoms are mostly cognitive but last for days together. I have tried most supplements/ medicines which have been helping people here - Niacin, Garlic/Fenugreek, Goji Berries, Vitamin D, B12, Zinc, Magnesium, Omega 3 a lot of ayurvedic aphrodisiacs from India (Shilajeet, Ashwagandha (Ginseng), etc), Ibuprofen, Paracetamol, tamsulosin, Anti-allergens but nothing worked at all. Have been on different SSRIs and SNRIs for 6 months when I hit severe depression due to my condition and work pressure. Post-depression and stopping use of anti-depressants, I hit a high for a few weeks where I felt I was cured, the confidence was back but it was temporary until I swayed back to this debilitating condition.

A year back, I stumbled upon diclofenac 50 mg which I tried Post O. It gave me 30-40% relief in the cognitive symptoms. But I was cautious about its use since my family doctor said it has bad side effects. I never had more than 2 tablets in a week. A couple of months back, I had multiple orgasms in 3-4 days and I was feeling very weak. Diclofenac wasn't available then so I tried Diclofenac (50 mg) + Paracetamol (325 mg), I felt a little better and much better the next day ( 40-50% healing). 1 day after taking this combo, I had another O, and woosh, I didn't even get 10% of the symptoms I usually get. It felt so great, after 10 years of suffering to have an O without any symptoms. Since then I've been using it Post O and it helps me to cure to 50-60% if not 100%. Haven't really tried it Pre-O, except the first time when I was 90% relieved. My O these days are mostly nocturnal emissions during sleeping (1-2 times a week) so can't really try it Pre-O. But this drug has been a real lifesaver for me. I still have apprehensions over its use since it is not a cure and avoid taking more than 1 tab in a week.

This combination is supposed to work for inflammation, so POIS is causing high inflammation in my body somehow. But I've tried taking ibuprofen which wasn't useful at all. Please help if someone understands the mechanism here, even if slightly. I may be on the verge of finding the cure.

Allicin did not help me at all and just gave me heartburn, but aged garlic extract helps the most of anything I’ve tried.  The chemical makeup is different from regular garlic due to how it is prepared, and there are several studies documenting aged garlic extract’s ability to lower inflammation and immune system activation.

I take it in the morning and evening, and it takes me from 20% functional throughout the day to around 60-75% functional, which has been amazing for me.

Not perfect and some days are harder than others, but it is helping me live life more fully.  Hope this info helps somebody.
The cause is probably a combination of autoimmunity and SASP leading to excessive oxidative stress and lipid peroxidation. Antioxidants, testosterone, NO and norepinephrine boosters, ERbeta, sigma-1, SIRT-1 and dopamine agonists, PDE4, PDE5 inhibitors and CD36 antagonists are effective.
