Do you have any family members with health issues?
as far as i know, in my family there is no significativ problems :
no allergy problems.
no thyroid problems.
no hormonal problems.
no cancer problems.
no heart problems.
but i could miss something that i dont remember or just dont know.
the only thing i remember are varicose veins in leg for my father and grand mother that i dont have for the moment.
in my case i think the problem comes from when i was a teen.
i lived in a hard and bad family environment (...) that result in traumatic stress.
and that stress seems to causes malfunction hormonal glands, and for me specifically adrenal fatigue and low cortisol production.
that i think is the main cause of my POIS at this moment.
i know there is a lot of different case here,
but if i refer to the forum it seems lot of people are sensitive to stress/depression and have a hormonal disorder (testosterone, cortisol, acth, low or high)
and I don't think it's a coincidence
I also know that there are also many cases where people have normal hormone levels,
but I think there are things that could explain that
first thing :
it seems that some hormones work
in combination with others hormones vitamine or protein.
this means we could have a normal hormone level but if we don't have the complementary substance it results anyway in a malfunction that could be the same as we got a low level.
(i dont have any example yet for this case, need more research)
second thing :
in the same way it seems that some hormones work
in opposition with others hormones.
this means that a hormone can still have an impact on another one even with a normal level
eg : in a stress situation cortisol (stress hormone) stop or block production of testosterone.
and in the forum there are some case with low testosterone but with normal cortisol level.
third thing :
in an opposite way it seems that some hormonal glands are
dependant from others hormonal glands.
this mean that some hormones/glands with high or low level that we think got a problem dont have it and the problem comes from an other gland/hormone.
eg : the testicular glands which produce testosterone and the adrenal glands which produce cortisol both appear to be dependent on the pituitary gland which produces ACTH which is used to communicate with them and tell them to produce or not.
and in the forum there are case with low or high testosterone and low or high cortisol, but there is also some case with high acth.
fourth thing :
it seems that some hormones could be used as a gaz that are only used when needed (eg :cortisol)
this mean that a hormonal level can be seen as normal and we may not notice we got a hormonal disorder like slow production/refill process until a breakdown/crisis cause by an orgasm
orgasm that involve a lot of others hormones (oxytocin serotonin dopamine etc...)
(need more research on those other hormones and their function and interaction)
conclusion :
it could have been many case where we dont notice we got an hormonal disorder
or a disorder that depend from another one we dont notice too
because of how all those hormones work and how they interact with each others
so my first thoughts and theory at this moment and after reading a little bit on the forum are very simple :
stress cause different body disorder : muscular pain / stomach burn / allergy / etc ...
but very high stress (traumatic stress / chronic stress) cause deeper disorder like hormonal disorder
and this hormonal disorder can result in POIS

since all those glands/hormones seems to interact with each others we can easily imagine if just one of them is malfunctionnal the whole chain can break with the same result in POIS.
the difficulty could be that in some case hormonal level would be normal and we need to check other thing or use other method to detect and reveal it.
i think for sure some others "poiser" already explore this theory but maybe not considering some point i tried to show with example (at least i hope)

for sure what i said need some verification and deeper research but this is my initial thought

this is because in my case cortisol is the starting point
but as i tried to tell you, for other people the starting point seems to be the testosterone or acth or any other hormone turning around that could be involved in the process.
the point would be to have better knowledge of how all those hormones interact with each others and which ones are crucial in the process including other substance like vitamin or proteine that can be needed
to confirm this starting point i think i will also need to explore how work others sexual hormones involve in orgasm like oxytocin, serotonin, dopamine etc... and how they could interact with cortisol testosterone or acth
