Hello everyone!

I found this forum at the beginning of this year and after a few weeks of being a passive member I want to share my thoughts about my case and POIS in general as that might be helpful for others.

My story:
I don?t exactly know for how long I have had POIS now. All I can say is, that I tried NoFap at the beginning of 2018 and after a few days I felt like I had superpowers, just as it was promised with doing nofap. :D I was discussing this with a few of my friends back then and they never experienced such benefits from abstaining. I reduced masturbating from then on, as I felt it was veery beneficial for me.
2 years later I started dating a girl and after some time we started having sex. That?s when I really felt that there was something wrong with me as my eyes started burning like hell after every time I had an orgasm during sex. I also had the full spectrum of mental symptoms but I didn?t connect them with orgasm back then. After some time of tapping in the dark I found out about POIS by googling and everything made sense! That was about a year ago.
I went to an urulogist who has never heard about the syndrom and said that I need to see an psychologist. So that has not helped at all. Finding this forum was a lot more valuable though. I believe I now know the root cause of my POIS.

Fructose Intolerance/Leaky Gut:
I am pretty sure I am intolerant to fructose. I never really realized I had digestive problems until I read some threads in this forum regarding diet. I?ve had a lot of bloating, gas and sticky/voluminous stool before starting a very low fructose diet. I started this diet around 8 days ago and 2 nights ago I had a noctural emission. And guess what: My symptoms were drastically reduced to what I usually experience during an episode of POIS, even though I have to say that symptoms after NEs in general are less harsh for me then after having sex or M. I realited that I have mild POIS symptoms after earing a lot of refined sugar/fructose. So bloating and (mental) symptoms of POIS are definetly connected in my case.

I believe that because of eating a lot of food that my gut couldn?t handle (fructose, maybe some orger stuff wich I?m not aware of right now) it became leaky. Because of that POIS developed afterwards. The connection might be the vagus nerve.

As I said I started a no/very low- fructose-diet and I will do this for at least another week or two. I will try out how I feel after an O after that time. After that time I will also try to bring back some fructose into my diet and see how I can handle it.

Some other information:
- I also can?t handle sorbit as it makes me very nauseus
- Nicotinamid doesnt work for me at all
- Cetirizin seemed to help a little (maybe placebo)
 - Fenugreek gave very little remedy
- When I am thirsty I get VERY angry and agressive until I drink water (is that normal?)
- I started supplementing basically all vitamins + zinc, magnesium, copper, iron, curcumin

I have good experience now with taking probiotic drinks (Actimel, Yakult) and others have mentioned that (quality) probiotics help them.

Let us know of your progress..
Not to discourage you, but: I think there is a lot of lifestyle/food choices that help people, but they (usually) only fix x% of POIS.

Hi Berlin! Thanks for your answer!

I read about fodmaps and low-fodmap-diets. I just feel that I don?t get any symptoms from Gluten and Lactose. So I believe it is really just the fructose that I have to reduce.

Probiotics are a good advise. I thought about taking them but wouldn?t they be harmful for me, if it was SIBO that I am suffering from?

I ate some fiber (oats) yesterday evening and I feel bad today.. couldn?t that be a hint that I have SIBO?

Peculiarities/miscellaneous:
  • The fiber free diet I had to eat for a few days prior to the hydrogen lactose breath test gave me better stool

Then don't eat oats :-)
I don't eat them either (+ no milk cream, no garlic, no onion).

FODMAP does not mean you need to have intolerance to ALL of the FODMAPs.

SIBO is IMHO just term (mis)used to mean "dysbiosis of bacteria somewhere in the intestinal tract" or "for some reason can't digest a certain food", it does not have to be in the small intestine.

Some people try to take probiotics, some try to take enzymes, some try to increase their stomach acid.

I think POIS also has to do with histamine in some way. I am taking 2 ceterizin every day noe and I feel much better. I am still on a very low fructose diet.

I had an ?o? yesterday evening and I had symptoms afterwards, which weren?t very severe though. This morning I felt slightly better but symptoms got worse during the afternoon. In the evening they went away completely. Usually symptoms lastet up to 7-10 days when it was really bad.

 I think I am on a very good path.
I will start probiotics tomorrow. I will also test in what histamine through diet plays a role in my symptoms.

Thanks for sharing.
Your method combines two approaches that have each been successful for other members, and are listed in my POIS Types Chart:  use of antihistamines, and elimination diet of a nutrient you are intolerant to.

I wonder if the fructose released during ejaculation could maybe induce some allergic symptoms for those who have fructose intolerance.

* Fluid from the seminal vesicles accounts for approximately 70% of semen volume. The seminal vesicles are the source of fructose in semen. Fructose is used by the spermatozoa as an energy source.

Neem may reduce fructose in semen, although it could also adversely affect sperm quality.

Azadirachta indica adversely affects sperm parameters and fructose levels in vas deferens fluid of albino rats.
Azadirachta indica treatment for 24 days in albino rats resulted in a decrease in the total sperm count, sperm motility, and forward velocity in vas deferens fluid. The percentage of abnormal sperm increased and the fructose content decreased. As diminished levels of fructose parallel androgen deficiency, we conclude that reduced androgen levels resulting from the antiandrogenic property of A. indica leaves probably influences the physiological maturation of sperm.

Neem was actually beneficial for some members, although there are other theories why this could be so.
The cause is probably the senescence of sexual organs and resultant inducible SASP, which also acts as a kind of non-diabetic metabolic syndrome.

Some alpha-1 blockers can also decrease seminal fructose.

The results of semen analysis following alpha-1 blocker administration, including the ejaculatory volume, sperm concentration and total fructose level, significantly decreased under tamsulosin administration (Figure 2).
To determine whether the ejaculatory disorder depends on the reduced contraction of seminal vesicles, we measured the fructose level in the seminal plasma, because fructose is known to be secreted only in the seminal vesicles. In our study, the total fructose level was reduced in the tamsulosin group, especially in patients with low ejaculatory volume (80% decrease from baseline), which suggested the existence of seminal emission disorder.
This finding also supports the theory that alpha-1 blocker induced a loss of seminal emission but not retrograde ejaculation.

The seminal fructose may stimulate taste roseas of the urethra and glans penis via pudendal nerves. The present study describes a new neuro-morpho-chemical mechanism of orgasmic sensation with its neurosurgical aspect.
We concluded that the occurrence of penile erection may be triggered by urethral taste roseas stimulation by fructose injection. It may be related with male orgasm. This finding may be a gate for the explanation of neuronal and anatomic mechanism of orgasmic sensation in humans.
If fructose concentration decreases due to disease, then infertility and orgasm disorders may occur. In this study, we found that seminal fructose is required to stimulate taste roseas and to support the generation of orgasmic arousal; orgasmic sensation may be facilitated or intensified by pudendal nerve. Intact pudendal nerve is important in the generation of erectile penile posture. Thus, we argue that stimulation of the taste buds by seminal fructose in the penile urethra may play a corroborating role in the erectile posture of penis.
