Hi,

I am 34M, I have been for the past 10 or so years battling every winter with unending bouts of sinusitis/rhinitis/sore throats that don't usually respond to antibiotics (even the fluoroquinolones) but just time and rest.

During those wintery bouts I have noticed a few interesting patterns that may be related to POIS:
- I get alot more in the mood before and during those bouts of illness, even if I am exhausted.
- High anxiety, poor to very poor sleep.
- Higher body temperature but not usually into the "real" fever range (38-38.2C max usually) (where I live fever is defined by body temperature above 38-38.5C at rest, with no stress etc for at least a few hours/day, usually in the evening or end of afternoon, measured rectally in a room at normal ambient temperature around 20C) please note I dont usually get shivers either just a constant feeling of warm, looking to cool down.
- Eating pizza I noticed a few times during said "sinusitis" makes it worse the next couple days.
- Orgasms during that time feel like agony. Muscle cramps, feeling even more achy (increase in flu like symptoms), a possible increase in body temperature for hours after, nausea, diarrhea, shakiness.

A few ENTs have diagnosed me with "chronic inflammatory or allergic rhinitis" as the initial source of the problem and I have been put on non sedating antihistamines and steroid nasal spray beginning of this summer as a test; my nasal passages cleared up real fast, no more thick gunk and normal sense of smell returned; it didn't however prevent relapse (I am currently ill again). There are no obvious structural issues aside from inflammated and swollen concha, but still within "normal range" on airflow resistance testing.

Out of the wintery season I am usually fine (aside from anxiety and depression); but orgasms still sometimes are problematic with the following symptoms (to be compared with above):
- Shakiness/weakness (sometimes nearing syncope, talked to a cardiologist about that one, stress test didnt think it was related)
- Cramps
- Diarrhea
- Sudden Flu like symptoms
- Nausea
- and... AND... a short lived feeling of nose gunk flowing down my throat just like when I have "sinusitis/rhinitis" making me want to spit or somehow gargle something to get rid of it.

Sometimes these symptoms start even before orgasm, forcing me to abort because I know what will follow. I can feel them creeping in.

Also my skin is usually dry, and I do have the beginning of mild rosacea (but it runs in the family)

To me, all this screams transient histamine hypersensitivity

I have similar symptoms. In addition to the sinus issues, I also have lots of chest congestion.

Something I forgot to mention, is when an intense POIS strikes, in the couple minutes after orgasm, I get absolutely disgusted by my own semen.

The smell, the texture, to the point of nausea. At the same time my body aches and for some reason my tongue tingles briefly and my face feels flushed (if I look in a mirror I am indeed quite red)

Oh, btw I do NOT get any rash anywhere semen happens to land. Its just the act of orgasm/semen release makes me sick The closest thing I could compare it to is brief but extremely intense nausea; but too brief to result in vomiting.

I would describe it as the strongest feeling of disgust I have felt.

To me it sounds like massive vasodilation secondary to a surge in histamine... or an adrenalin/vasopressin crash... not sure.
