Lower ab swelling

As soon as I get hard, both side of my lower abs around my appendix but on both side rather than just on the side of the appendix, get swollen. It feels like two balloons are inflating. I am wondering if anyone else experiences that and what theirs feels like. I am thinking its gas or my prostate swells up only when I am hard. I dont get it. Also when Im hard I tend to clench my anus. Does anyone else notice that?
POIS sufferer for over 3 decades. Has progressively gotten worse over the years and I became completely disabled around 2011. My case of POIS is very severe.

I notice this after orgasm and for several days afterwards.  Also have trouble taking deep breaths as well.  Very annoying.

I think it's inflammation

I usually experience tension in my lower back, especially where the appendix and cecum are located. I have now discovered that when this tension increases, then i get a wet dream/night release
Thank you community! I am no longer alone, with “unreal” symptoms.
I have hypothyroidism & constipation (toilet every 3 days or more). Worst constipations = more pois, & lots of mucus in the stools, or just mucus coming out.

Its not inflamation. Its just worsening of central neuropathy in the brain that sends signals to muscles around appendix.

Walking every day heals this fealing.

This might be better with the Fodmaps diet!
