POIS Questionnaire For Review

Hi All,

Please find attached a the draft POIS questionnaire provided by Barry R. Komisaruk, Ph.D. Due to technical issues on the forum we have lost or is hard to find the previous discussions including what other categories we would like to add to the questionnaire. So may I please ask (on behalf of Daveman - forum Admin) that you provide your suggestions again now by following these steps:

1) Download the most recently added attachment in the topic
2) The original draft is on the "base questionnaire 1" & "base questionnaire 2" sheets. Please review these first before adding any suggestions (I know that sounds obvious). Note, the base questionnaire sheets are non amendable so you won't be able to make any changes them.
3) Any suggestions to add to the questionnaire should be added by completing the "suggestions" sheet
4) Upload the updated questionnaire to a new post on this topic adding the date to the file name

FB, thanks for re-formatting questionnaires so they're much easier to work with now!

This is incomplete and needs more... I hope this helps being that I experience extreme symptoms and can always add more but, these are musts for me because this flu/allergy stuff is just the beginning and mild but can get the point where I cannot sweat or regulate temp at all. When things get extreme, cognitive, acute pain and muscle tension become the worst.   

Digestive: Heartburn, Cramps, Vomiting, trouble swallowing

General flue like: migranes, inability to sweat/fluid retention

Allergic: swelling of skin and tissue, skin rashes, asthma

Cognitive: tinnitus, ticks, confusion, neruopathy, mania/mood swings, delusional though and numbness

Pain: Anal/rectum, spinal, and pelvic floor

Tried desensitization for 1.5yrs & was unsuccessful (POIS worse at 1/1000)

 Exercising- (running/weights/situps) Low sugar diet. Supplements- limited success.Meds- Oxcarbazepine/Buspar (past-Depakote10yrs)

Also I forgot a couple... Exhibit A OCD (checking the POIS forum 15mins after I logged off..)

There should be a sexual dysfunction category as well: ED, frequent erections, high and low sex drive depending on where you are in the POIS cycle.

Mild anal bleeding

palpitations

hypersensitivities to light and sound goes w/ the headaches/migranes (difficult watch tv, comp, drive at night....)

And my favorite

Acute pain while sitting

One that will prob not wk on the survey but is certainly worth mentioning is a the bizarre propensity toward creative skills/thought while in a more acute pois phase (day 2-3 for me) (I should note I have constant symptoms and may be at the more extreme end of the spectrum; I don't know)( no reprieve and symptoms even upon going #2 to bathroom at this pt).
