Personally I have had testicular trauma in the past. Now that I think about it it was around the time POIS started for me. It was at work and I had to take a large step while I was wearing tight pants. I remember I had severe pain for several days, so bad I could barely sit down. Even today I feel some dull discomfort in the plumbing from my right testicle into my body. This seems to get worse with orgasm. I've book an appointment with a doctor to see if healing this damage is possible.

It mentions a cytokine-driven immune response. I've seen some of the very smart people on here talking about cytokines and the immune system a lot. My theory is the damage to the tube from my right testicle has allowed sperm to enter my bloodstream and with each orgasms my body has built up antibodies to the sperm. Over time my body's immune reaction to sperm or these antibodies has become very intense. I would equate my POIS as the feeling I have when I'm coming down with a bad cold or the flu. In fact for a long time I just thought I caught a lot of colds, before I realized the timing was linked to my orgasms.

Some ideas on how to treat this:

-Vasectomy. I know some people here have tried this without success though. If the tube was tied before the leakage point theoretically that should stop sperm from getting into the blood stream. I'm not sure if that's how vasectomies even work though. I've heard some just sever the tube and leave it open.

-Vasoconstrictors. The best treatment I've tried so far is Allegra-D which has Pseudoephedrine. I'm not sure but possibly it can also constrict the damaged tubing prevent sperm leakage. Or perhaps it constricts blood vessels enogh that the sperm can't get in. Maybe that's why it helps me much more than regular Allegra.

-Immunosupression. Perhaps a dangerous thing to do for health reasons.

-Reduce systemic inflammation. This is more treating the symptoms than the cause, but by lowering total inflammation with things like improved diet, improved gut health, improved microbiome, exercise, removing other allergens or toxins from your environment or diet, vitamin supplementation, one could more readily absorb the inflammation caused by orgasm.


Anyways I'm sure these ideas have been discussed on here before. I just wanted to see if anyone else has experienced testicular trauma or perhaps one of the other risk factors for antisperm antibodies listed above?

Body aches and pains, severe fatigue, brain fog, headaches, poor appetite, low mood, irritability, depression, and anxiety.

Yeah but it doesn't sound like flu like symptoms. The auto-immunity theory on POIS has many flaws. One of them is that the brain symptoms aren't related to the autoimmune response in the testicles.

Neuroinflammation occurs only when there is an infection in the brain, an injury or Multiple Sclerosis.
Also the symptoms seem to be confined to the limbic system and the vagus nerve.

I think that in a lot of us here happens that we hawe a large
amoun of histamine in neurons of hypothalamus like in ADHD.
(from where it mediates AROUSAL and atention)

Yes, such as in Multiple Sclerosis where the body attacks the sheets on nerve fibers.

The thing is, it seems that vasoDILATORS help for POIS, so I wouldn't agree with the theory that vasoconstriction would help. For me, for example, alcohol or garlic make a serious positive impact. I should try viagra, a doctor recommended it to me (plus, it would help against the PE).

My mother feels better on a little bit of liquor but too much gives an opposite effect. I think the body might be stuck in a sympathetic state after ejaculation and doesn't adjust properly leading to increased cardiovascular tone plus inflammation. Carbohydrates might worsen the situation because it's vasoconstrictive.

But I have to add on the topic of testicular trauma that the girl I had sex with the first time my POIS appeared did hit me unintensionally with her elbow in the testicles and I recall feeling some pain and telling her something like "Please be careful." The pain disappeared very quickly but POIS started for the first time the next morning. This is why I cannot discard a hypothesis of something mechanical having taken place in the testicular region. Nevertheless, nothing unusual was seen on an MRI.

Drop, I also agree about inflammation crossing the blood brain barrier from body to brain. All my research says it does, and specifically how gut inflammation from a poor diet can cause brain fog and cognitive impairment.

This is one of my 3 theories of POIS as it relates to my case: testicular trauma.

Shortly after birth, I went through a surgical procedure that I’m sure created some
testicular trauma, thereby potentially making me more susceptible to developing POIS.

No, I don’t recall the procedure ;D ;D
10 years of significant POIS-reduction, treatment consisting of daily (365 days/year) testosterone patches.

TRT must be checked out carefully with your doctor due to fertility, cardiac and other risks.

40+ years of severe 4-days-POIS, married, raised a family, started/ran a business

I have had testicular trauma when I was young while playing sports, many times but so have many other guys. When you think about a theory and you realize that many other people could have had the trigger you suspect then you have to think why don?t more guys have POIS. I have had POIS since I started orgasming. Over 3 decades it progressed to one the most severe case of POIS. In the testicular injury theory why don?t all the other guys with that same injury have POIS. Also why is there subtype and wide variations of symptoms. The main thing we have in common is we feel unwell after orgasm, but we don?t share symptoms variations and we don?t all share the things that help.

As far as antisperm antibody that is interesting but if you notice those antibodies are not making men sick. So it?s not really a theory because almost every autoimmune illness has antibodies but the problem is finding the one that are unique. For example many autoimmune patients have high ANA, AntiNuclear Antibodies, but those are not specific to any one autoimmune illness. It could be some minor hormonal cell that not discovered that is the antibody and it might not me the actual sperm.

I would not recommend experimenting based off your theory. But there is no harm in our community having better tests on our reproductive organs.
POIS sufferer for over 3 decades. Has progressively gotten worse over the years and I became completely disabled around 2011. My case of POIS is very severe.
