Preventing POIS by Preventing Ejaculation
« on: September 19, 2011, 04:29:51 PM »
Hi

Ejaculation causes an autoimmune response in me. So prevention is the only answer.

~~~~~The Story~~~~~~~~~
POIS started for me at puberty. POIS manifest itself in joint inflammation that causes pain and that causes depression.
I notice the link between ejaculation and pain from the start but I couldn't believe it. For several years the only thing that stopped the inflammation and the pain where antibiotics.
Next I found Naproxen it worked, it stooped the inflammation.
I took 250mg at start then 500mg and now 1000-2000mg. The autoimmune response is increasing and the body will stop responding to NSAID, antibiotics etc.
~~~~~~~~~~~~~~~~~~~~~~

And like the other autoimmune disses I don't think that in the next 10 years there will be a cure. So the only thing I can do is to prevent POIS by preventing ejaculation.

Methods :
Disabling semen production - don't exist
Preventing the spinal reflex responsible with ejaculation - possible with antidepressants (side effects) or by elector stimulation (not practical to carry a device everywhere).
Preventing stimulation - local anesthetics, practical but opioid have side effects currently searching for a non-opioid local anesthetics my favorite and the only one that can be developed for use in less than a year.

Local anesthetics: numbs the penis

Lidocaine spray 10% one spray on the most sensitive parts works for 30-40 minutes after this one subsequent spraying will increase the total time to around 90 minutes, erections quality is slightly decreased.
Side effects swollen penis takes 3 days to get back to normal lidocaine is the safest of all opioid anesthetics and it has side effects.

I have not found a replacement but I didn't searched too much after a new one, new non opioid anesthetics are promising but still in test phase.
A non-opioid local anesthetic injected in penis with a needle free syringe is the best and fastest way of prevention.

Not feeling with my penis is not a problem, I want to find a replacement to lidocaine that doesn't kill me  in the next 6 months.

Hi Freedom, welcome to the forum.

Have a look around, I think you'll find that we're probably closer than you think to a final soluton.

Take it easy with Naproxen. It's really hard on the stomach and liver. Especially at your doses.

I guess the biggest problem with ejaculation prevention are the Nocturnal Emmisions. They sneak up on you and BAM, in POIS again.

Look at the help that Niacin can give you. Also "saw palmetto" reduces the sex drive.

Anyways, have a look around and see what seems to suit you.

WITHOUT RESEARCH THERE WILL BE NO CURE!
Sessions 5 to 9 days, mostly Flu-like, joints, digestion problems, light cognitive.
Niacin has changed my lif though, now 1 day MAX.
Somewhere in this interaction with Niacin is the answer!

Hi Freedom, i agree with Daveman when he says that you have to try less aggresive methods. I suggest to look the thread of Xantinol Nicotinate and try Niacin of fast release with flush. It´s changing my life certainly for the better, and POIS has been inexistent for me except for a few moments in the last fifteen days. Give it a try.

hey. i see you have joint & inflammation problems. Fenugreek + garlic helps me a lot with that. Saw palmetto works pretty well also. I tried naproxen for a few weeks and it had no affect on me.
Taking ginger tea, no wheat, fenugreek+green tea/garlic, saw palmetto, niacin, boswellia, huperzine, B complex and nutmeg. See my treatment summary post for more info: http://poiscenter.com/forums/index.php?topic=81.msg3513#msg3513

also there was a drug called silodosin that is known to often cause a dry ejaculation. I think 2 members here tried it and it seemed to work for one of them, but had a side effect of making them tired all the time or something.

I've suffered from POIS for many years, but I also have another problem: if I don't ejaculate for more than a week i start getting joint pain, neck pain, back pain... So for me, preventing ejaculation or giving up sex or masturbation altogether don't seem to be valid options. Am I the only one who gets these symptoms?

It's very interesting that you mention this! It's not something that is mentioned here often, and I think it's because it's kind of hidden by the fact that most don't make it beyond 5 days or so to have another orgasm.

But I've noticed it with the niacin. Niacin has me clean, basically "POIS free", but I've notice too, especially because I am in a cleaner position to see it, that if I go more than a week without sex, that "Some" of the symptoms show up without ANY stimulation.

I think this might also be the cause for some who NEVER from POIS even if they abstain forever!

What I think, is that there must be a build up (of semen?) and the system proceeds to dispose if it internally, just as it does with someone who has had a vasectomy. So even though we do not have and orgasm or even arousal, we may feel effects of POIS.

In my case, the effect is quite light however. And I usually don't go that long without sex, so I hadn't really noticed it until recently.

An interesting point.

can the body release semen why you pee or right before you pee?  I was just wondering how body gets rid of semen if you dont ejaculate.
I know those people that dont ejaculate/ supposedly dont have NE, their body gets rid of it somehow.

The body just absorbs / digests it.

And it is eventually expelled through the urine. (But not as semen at this point).

Thanx a lot Daveman for your input above. I was actually thinking the same thing.. i.e. that the system disposes of unused semen internally, and that this might lead to POIS symptoms.

One observation from my own experience that I'm excited about and that I would like to share here is that if i ejaculate more than once (twice mostly) in a session i get a notably lesser degree of POIS symptoms than if I did it only once. I hope this might work for other people too. My explanation for it is that when the body knows u'r not gonna use the remaining semen it ingests it for recycling or renewing (not to mix the old with the new, kind of like batteries). But when u eject ur semen - or at least most of it - then the body would not have much to reabsorb - and the lesser absorbed, the lesser the symptoms. It might sound counter-intuitive, because we might think that if ejaculation causes POIS, then more ejaculation should cause more illness; but it's the other way around, I believe!

I don't know if anyone has noted that here before, I haven't read all the posts; but i hope it helps my fellow POISers :)

Yeah, I get this too. The symptoms are pretty light but they are palpable after 4 or so days. This is what I meant by "lingering POIS" in my previous posts.

Interestingly, it provides further support that it's something about semen (and its productive process) -- not orgasm -- that causes our symptoms. I've been taking prescription anti-inflammatories to deal with this, and the results i've gotten are actually quite promising for you. The anti-inflammatories do significant work on my mental symptoms -- although i'm still trying to decide HOW significant that work is -- but they're most effective in combating my physical symptoms (swelling and discomfort in my face, around my eyes, and in the bridge of my nose). The drugs make all that inflammation just "dry up."

You should think about asking a doctor for Indomethcin or Celebrex. I take 1 pill a few hrs pre-O, and 3 additional pills over the course of the next 1.5 days.
POIS since I was about 15. 1.75 years of desens and I'm now about 80% POIS free. Still working through best practices for maintaining my immunity and administering my injections with my doctor. Email me if you have tips or questions!

In dealing with the lingering symptoms, I just kind of take it "as needed."

Egordon, what dosage are you taking? I tried Indomethacin a while back when the doctor thought I suffered from post-coital headaches. This was a year or so before I discovered the POIS forum and all of the other sufferers. Have you noticed a difference between Indomethacin and Celebrex? Your description of "swelling and discomfort in my face, around my eyes, and in the bridge of my nose" is identical with my own symptoms. I couldn't have put it better myself. It's almost as if the bones of my face are swollen -- it feels very tender, sensitive, and painful. I'm excited to hear your response!
Experienced POIS since 2002.

Hey Hoping,

I actually was initially prescribed indomethacin for the very same reason! Because I thought I had been wrongly diagnosed, I didn't give it much of a chance the 1st time around -- I only took it for a day or so. Recently, though, I tried it again and got decent results. Indomethacin gives me asthma, though, so I take Celebrex. I take a 200mg pill about every 10 hrs for about 1.5 days post-orgasm.

Have you found that it works for you? If not, do you use it with niacin? I didn't really see major results until I began using it to supplement my niacin treatments.

Egordon, Unfortunately, I can't remember the dosage of Indomethacin I took. As I recall, it didn't seem to do me much good the first time around. When I finally connected my symptoms to to orgasm, I briefly tried it again with little relief. I experimented a little, taking it before and after orgasm. It was a while ago, so my understanding of POIS was VERY limited. I may be willing to try it again.
It was before the whole 'niacin craze' took hold in the forum, so I never got a chance to try them together. Is Celebrex the same sort of drug? I know Indomethacin is typically prescribed for arthritis relief.

Hey Hoping,

Celebrex is indeed a Non Steroidal Anti Inflammatory Drug (NSAID) just like Indomethacin. Like you, I experienced very little improvement when taking anti-inflammatories as my sole means of relief. But recently i've been pairing it with Niacin and have found that it makes Day 1 (post-O) feel like Day 4. This is a HUGE improvement for me because my job requires me to be able to articulate myself forcefully, make presentations and think quickly on my feet; I'm typically not able to do any of these things until Day 4 (even after having taken Niacin).

But I really only use anti-inflammatories because my job requires especially strong communication skills and POIS cripples those skills pretty severely. Otherwise, I'd probably consider the side effects too great and would avoid taking them.

I'll agree that the only thing I take besides the niacin if I really feel I need it, and that's usually a lot more in winter, is Iboprofen.

Between the the two on day 1 I have almost zero symptoms. After that I don't need anything.

Can I ask how/when/how much niacin you take? Likewise, how/when/how much Celebrex you take? I usually take Aleve to help alleviate symptoms and have actually had some success. It definitely makes things a bit more bearable, although I wouldn't exactly call it a "cure". I'm also leery ulcers from taking it for several consecutive days/weeks. I purchased some 100mg niacin a couple months ago and have experimented with it a little bit. I haven't had great success with it but plan on doing some more experimenting before forming an opinion on it. I agree, the social interactions (especially at work) can be killer! Actually, I can remember going to a job interview a couple years ago--before I really understood POIS--while symptomatic. I did my best not to have a panic attack and vomit on myself during the conversation  ;D.

Sure! I take niacin in the same fashion as most folks here -- about an hr prior to O. The amount of niacin I take varies depending on how quickly and how strong I flush, but its generally between 250mg and 400mg. As for the Celebrex, I usually take one 200mg pill soon after O (before bed, usually), and one the next morning (bout 10 hrs later). If necessary, i'll take a 3rd pill that night, but usually i'm feeling pretty ok after 2.

I don't much worry about ulcers for a few reasons. First, I don't use Celebrex everyday -- mainly because NSAIDs are so bad for you if you use them regularly! I try to only use Celebrex when I absolutely need to be on top of my game -- about once a week -- and shift my sex life so that it otherwise doesn't much interfere with work. The rest of the time I just use niacin, which does away with about 85% of my symptoms (... the rest aren't gone until Day 4). The second reason that I don't worry a ton about ulcers is that Celebrex is supposed to be a little better on your stomach than the other NSAIDS (although one time I took it with nothing but a banana and woke up in the middle of the night with killer stomach pains).

I'm glad allegra's working for you. I tried a number of antihistamines but never saw any results. Do you use niacin, as well?

Hope this helps!
