Tight testicles during erections

I have noticed the last 2 years that my nut sack get super tight when I am hard. My nut sack it almost flat its so tight and my nuts are very high up. I wonder if this causes my pois to be worse

POIS sufferer for over 3 decades. Has progressively gotten worse over the years and I became completely disabled around 2011. My case of POIS is very severe.
