liver friendly sollution because of Gibert Syndrome?

Hi there,

I'm new here, I'm 45 years old. And probably suffering POIS since puberty. I think this because since puberty when I started to be hormonally-active I always felt depressed and brainfogged and very tired after orgasm (ejaculation) and I still am when I do now these symptoms always continue for a couple of days up to a week (or more).
My libido was always quite low, sexuality was and is a thing I try to avoid because it is something annoying, but my hormones dictate me to orgasm once a while I never liked it to be ruled by my hormones.

As I said I'm in my 40's now and controlling my hormones is easier now then before (my libido is even less then when I was younger). My partner and I are together for 16 years now, I'm happy she has also a quite low libido, so sex is not really an issue we love each other... (we didn't have sex for 2 or 3 years I think). I still orgasm once a while (masturbation) but I try to avoid it (longest without was ?6 months I think). When I don't have ejaculations I feel so much more in balance and energetic...
But once a while I have to. I always try to plan it a bit beforehand so I can crash after doing but I've had it, I don't want to anymore.
So this is how I found the information about the POIS syndrome. I recognize a lot of things which is been said here on the forum I finally feel that I'm not alone in this and this feels good.
I tried the Niacine (flush) twice and it worked quite well for me the symptoms went back to 10/15% at the first attempt for the second time it was less effective 50/60% but this was when I orgasmed for the second time in the same week. (I normally O. once a month or less so that's maybe why..) But experiencing the first orgasm after taking Niacin felt really fine and promissing...
The only trouble is I can't use Niacin regularly because I have a liver defiency (Gilbert Syndrome). Using Naicin on a regular bases would probably cause serious liver problems and I don't want that either. I don't drink alcohol etc because of the Gilbert syndrome.
My question is: are there other forum member who have Gilbert Syndrome and maybe have another way to fight POIS in a liverfriendly way?

Someone on the facebook group(?) claims that Calcium D Glucarate helps when having POIS + Gilbert's.

I've never heard of this, but when I search for 'Gilbert' on forum quite some people seem to havt it. Interesting... at least from 23andme I think I don't have it (rs34983651 II)

When you say you have Gilbert's syndrome, how do you know it? Did you make a genetic test? Is it only because of the high bilirubin in the blood tests? Were you sick with jaundice or some related issue?

I have possibly Gilbert's too. High total bilirubin, conjugated bilirubin is half of the free bilirubin, the doctor said it's Gilbert, I don't have the genetic data to confirm. Never had any problem due to this, *as far as I know*.

Many people have high bilirubin on this forum. A few months ago I spoke elsewhere to someone who claimed to have seen quite many POIS linked to Gilbert's and had a theory about it. In short, problems to break down hormones, notably 3adiol, which then inhibits 5a-reductase and causes low allopregnanolone, GABA-A receptors downregulation, AMPA receptors upregulation and many other consequent problems. I have no idea whether this can be true. As for me, I don't seem to have inhibited 5a-reductase, as I have normal DHT.

Recently I have also read that Naltrexone, an opioid antagonist, could cause high bilirubin as a side effect, so I have wondered if bilirubin problems with Poisers could not be linked to the opioid issues hypothesized by the Chinese doctors in their 2015 POIS paper (see Jiang & alii, Postorgasmic illness syndrome in a Chinese man: no proof for IgE-mediated allergy to semen), and by other people. It may be equally dumb, I don't know.

This is the most important cue so far regarding POIS, it links together with other issues we experience like slower beard growth, muscle gain (less androgen output due to high Estrogen decreasing androgen output and too much T being converted into it and weaker 5 alpha-reductase and weaker DHT possibly from liver overload (good liver function ensures good Thyroid, 5 alpha-reductase, DHT output)
