POIS Research / Re: Great News - POIS Lab is Almost Ready for the Study !
« Last post by demografx on April 18, 2025, 01:24:41 PM »
Is there a timeline available? How long will they be piloting, when does the main trials start, when are results expected etc?
Last News Demo had is that this week, they received the last computer they were waiting for... but no Operating System installed, unfortunately. So the researchers had to reach for a Windows license and install it.  I hope this will be one of the last delays before the pilot study will start.
I have no idea how long will the pilot study last.  Maybe Demo will ask in one of his future contacts with them.  But as you can  see, any timeline is subject to change, depending on the problems encountered.
We have to be patient.  It is not a blockbuster study on diabetes, asthma, or overweight - POIS is classified as a Rare Disorder..  So, not a huge team in the lab to support our researchers, who also have themselves other duties and responsibilities to take care of.  But we are getting there, slowly but surely. Dr.L and Dr.P are very determined to get this project running as soon as possible :)

Thank you, Quantum!

"For the last 6 weeks, I've been taking milnacipran (Savella) before O. After O, there has been no brain fog - every time. My physical symptoms (dry hair, dry forehead, clicking knees) have also gone.

The milnacipran has an unusual side-effect - my Os feel much more intense, and last much longer. This hasn't happened with any other medicines I have tried, and I have tested many different medicines over the years. Again, this happens every time."

Interesting, this is what happens to me with the hops and Yucca Root combination. I see a mention of honey on the post too and I already benefit in a very general way with energy and mood with propolis which I take a capsule of every morning. I have a feeling that I am cutting on noradrenaline use while retaining higher levels after orgasm. When I hit the highest dose on vitamin C is when I was struck with the absolute worst symptoms even though my energy in a rather raw sense of the word was off the charts. The new combination of things also when the alpha acid is too high in the hops leads to feeling a bit too sedated BUT my energy levels otherwise are much higher the next day along with motivation and general ability to do more. They also have a terpene profile too with effects that may be playing into what is happening. After an orgasm when I'm feeling better I can also go for another and feel just fine as well as it being as pleasurable as the last but like I mentioned in my last post can be draining but that's probably just because an orgasm is one of the most complex things the body can do anyways.

OAT (organic acid test) results are in. I got GPT to interpret, but I am waiting on a consultation with my herbalist. In the mean time, this is what my OAT test is indicating (GPTs interpretation). OAT is commonly used for understanding complex chronic diseases.

1. Fungal/Yeast Overgrowth
Elevated Markers: Citramalic, 5-Hydroxymethyl-2-furoic, Furan-2,5-dicarboxylic, Furancarbonylglycine, Tartaric, Arabinose

Interpretation: Strong signs of fungal overgrowth, particularly from Aspergillus and possibly Candida.

Recommendations:

Antifungal therapy: Either prescription (e.g., Nystatin or Fluconazole) or botanical (e.g., oregano oil, caprylic acid, berberine).

Probiotics: High-quality, multi-strain probiotic (25–50 billion CFU/day).

Dietary adjustments: Avoid sugar, refined carbs, and fermented foods that can fuel yeast.

Consider follow-up testing: Mycotoxin testing for further fungal species insight.

2. Bacterial Overgrowth
Elevated Marker: Hippuric acid

Recommendation:

Digestive support: Probiotics, possibly antimicrobial herbs like garlic, neem, or berberine.

Reduce paraben exposure: Avoid parabens in cosmetics, lotions, processed foods.

3. Elevated Oxalates
Elevated Markers: Oxalic (88), Glycolic (87), Succinic (8.7)

Interpretation: Suggests fungal overgrowth and possibly issues with oxalate metabolism.

Recommendations:

Calcium citrate with meals to bind oxalates.

Vitamin B6 (50–100 mg/day), magnesium, NAC, and taurine to support oxalate processing.

Low-oxalate diet: Avoid spinach, beets, nuts, soy, and chocolate.

Antifungal treatment: Since yeast may be a key source.

4. Mitochondrial Dysfunction
Markers: High 3-Methylglutaric and 3-Methylglutaconic acids

Recommendations:

Mitochondrial support supplements: CoQ10 (100–200 mg/day), Acetyl-L-carnitine (1–2 g/day), Riboflavin (B2), Niacinamide (B3), and Vitamin E.

Investigate possible environmental toxins with a GPL-TOX or heavy metals test.

5. Neurotransmitter Imbalances
Findings: Low VMA, high HVA/VMA ratio, low 5-HIAA

Interpretation: Suggests low norepinephrine/epinephrine, dopamine dominance, and low serotonin.

Recommendations:

Support neurotransmitter synthesis:

Vitamin C (500–1,000 mg/day) and copper for dopamine beta-hydroxylase support.

Tryptophan or 5-HTP for serotonin (monitor carefully).

Avoid aspartame.

Consider genetic SNP testing (MAO, COMT, DBH).

Mood support: Use SAM-e, adaptogens (ashwagandha, rhodiola), or amino acid therapy based on symptoms.

6. Vitamin Deficiencies
Low Markers: B6 (pyridoxic acid), B12 (methylmalonic), B2 (glutaric), Vitamin C, CoQ10

Recommendations:

Start with a broad-spectrum B-complex with activated forms (e.g., P5P for B6, methylcobalamin for B12).

Add Vitamin C (buffered if sensitive).

CoQ10 for mitochondrial and antioxidant support.

7. Toxicity Indicators
Elevated Succinic and 2-Hydroxyhippuric acids

Recommendations:

Reduce or eliminate aspartame and exogenous salicylates.

Consider a detox protocol: sauna, niacin (Hubbard protocol), glutathione or NAC.

Look into environmental toxin testing (e.g., Great Plains GPL-TOX).

This post is incredible, I have never heard of OATs before but your results are incredible.
I have dealt with candida in the past and I most likely still have it, since I have a white coating on my tongue, which is known as oral thrush.
Its incredibly difficult to deal with in my experience, one cheat day and you basically go back to square one. I will write up about my experience when I have more time, I've managed to eliminate some candida from my body for short periods (couple days/couple weeks) in the past and it has huuuuuuge impact on my body, for me the biggest effects was clear eyesight (felt like I was seeing in HD without glasses) and once I had an insanely clear brain, the brain fog completely lifted for a bit and the previous time I got partially rid of it my oral health improved drastically it was like every taste and texture was 10x more intense, that time my oral thrush on my tongue was completely gone for a little while. I never experimented with PMO so cant tell you the effects on POIS.

Also, do you know why the chatGPT is advising to avoid aspartame?
Where did you hear about OATs? It seems geniune although results would vary a lot no? since your urine might get affected a lot by what you previous drank or ate? I did a hair mineral analysis test years ago but now I realised it was probably not very meaninigful, at least it told me I had low levels of toxic metals so that kind of gave me some relief, but the website I heard to get hair analysis tests I now realise was just quackery and the dude is just trying to sell you his supplements

POIS Research / POIS doesn't occur on this stack (inputs appreciated)
« Last post by Poisick on April 18, 2025, 08:59:31 AM »
So just some background my relationship with POIS and general sexual disorders has been rocky. I developed POIS in puberty around the time my dsyphoria also began to sky rocket (transgender female but currently not on HRT due to circumstances, so take that into consideration in how things may work differently in my body. Untested for intersex conditions but I have generally feminine physical features mixed with masculine with some sexual differences in how things work down there). This would be a constant struggle my whole life and peaked in severity in my mid 20's to my later 20's, I am currently 31. A couple years back though I began trying to grow out my hair and started saw palmetto and was also on lions mane at the time to combat my lack of energy which likely kindled the crash saw palmetto caused over a month landing me with a PFS/PSSD presenting syndrome. Full body numbness, genital numbness, no sexuality, no emotions, no pleasure/reward, no hunger/thirst, crippling fatigue leaving me nearly chair bound for months, slurred speech, extreme head pressure, ect-. You get it, it was pretty textbook and very bad. BUT when this occured I had lost my POIS along with my reactions to almost anything else at all but this left me with nothing. A couple years down the line and slow progress after throwing everything I could at the wall I recovered. BUT the bad news is the POIS returned with a vengeance after trialing a ultra high dose vitamin C experiment which knocked things back online again completely but in a very chaotic, dysfunctional fashion. The crushing feeling of constant anxiety, adrenaline, mood swings, and physical discomfort. I was going to comment on the noradrenaline topic too at some point as I have a feeling it has to do with this.

Anyways on to the recent findings.

Current stack - 2000 mcg methyl-b12, 100 mcg K2, 2500 iu vegan D3, 1 capsule propolis, 2 grams vitamin C, 1 capsule Yucca Glauca root (all in the morning, yucca was the most recent addition). A couple mugs of black coffee (morning), a couple later (usually around 1-2 PM), a cup of plain cistus incanus tea (around 3 PM) followed by a cup of green tea (often right after the cistus)

Story - So after a disaster of fluctuating symptoms moving past my baseline of 2 grams of vitamin C daily where I found if I took another couple later at 3 PM I would get more energy and increased sensory clarity I decided to take it further and see what would happen if I took 2 more grams for a total of 6 grams in a day later at 9 PM. This gave me much more physical energy but the stimulation was so overwhelming that I felt like I was going to have a emotional mental breakdown and anxiety attack most hours of the day. This also caused horrific insomnia that didn't taper off much upon stopping essentially leaving me stuck in a high strung adrenaline overload state where I couldn't really do much of anything but go through the motions trying to minimize stress because I couldn't handle any of it at the time and the smallest thing that came up would make me feel like I was going to lose it. If I had an orgasm or got aroused it was a full blow POIS episode for the next 2-3 days with some taper off.

So I thought back while browsing one of the PSSD forums about people who felt better with hops. Desperate for any relief from this insanity I ordered the Biokoma one's on Amazon. And one night tried them. They were the first night ridiculously sedating but relieved me of all symptoms alone saved by a background of high anxiety that would amplify more in the aftermath of a night of making the hops tea but would leave me much more functional and handling things better. The next night I tried again and these effects with the sedation were less than the night before but I really liked what this was doing for me so I kept going. The next night after that I was clearly becoming sensitized to some effect of them but my reaction was more stable and I felt better with them then. The night after that I tried adding more (it was 2 tablespoons of dry flower at first, but I added 1 more) and there was ironically a more mild stimulating effect but I slept even better and on the rebound the next day I felt better. The days after a night of hops my senses were so much clearer, my brain worked better (I can read, enjoy things easier, socialize better with far less vocal dysfunction yet at times I was slower speaking under the hops afterglow), and my anxiety was greatly lessened and I could go outside without feeling like I was going to have a complete meltdown. It wasn't perfect like I said with the background anxiety and if I orgasmed I'd still be prone to POIS but a lessened symptom version. The typical symptoms would all ramp up but be somewhat less life ruining. The hops also increased my libido a fair bit and orgasms felt much better and full bodied.

There was another supplement after this that came into play where things clicked. The digestive issues wiith the POIS episode would be ridiculous still, in fact that hops made my guts even worse in some ways on their own. On the Pheonixrising forum I saw someone mentioned that Yucca helped them with their guts so in a shot in the dark I decided to give it a go. The first day I used it (1 capsule, Secrets of the Tribe brand Yucca Glauca root) I noticed an increase in baseline energy without stimulation and better social abilities and vocal control. It also immediately settled the gut issues. My libido also sky rocketed at some points in a somewhat uncomfortable fashion though not sure if that was just a residual effect overlap from the hops. The first day with it was the first day I got off the hops to see if there was any rebound/dependence that occured so that night I had none of those. The next day I took the Yucca again and the same thing occured but the benefits were more persistent with one downside where they made me feel a bit too amped up at times even though everything was more functional. If I orgasmed with just them I'd still get POIS but like the hops these symptoms were reduced. With the Yucca alone the reduction was more in brain inflammation feelings, spatial processing problems, and vocal dysfunction. That second night I decided to try the hops again and this time with the Yucca on top there was something somewhat different going on. I felt FAR more stable and could enjoy everything much better. The benefits of both together combined, the next day I felt much better. When I orgasmed under both of these which I repeated twice counting this morning to confirm what I thought was going on I noticed that on this combo with my current base stack I experience no POIS. A little tired after and might get a little nasal drip but that's it. I can also just keep going and the sex will be just as pleasurable but obviously I would simply keep getting more exhausted so I don't tend to do that as much as I want to on this stack.

Last night I tried a hops strain with much higher alpha acid (not sure what Biokoma brand AA content is but this one was clearly far higher) content of 10.6 percent. This was way too much and practically knocked me out leaving me dreaming while awake for hours until finally hitting the pillow and having ridiculous vivid dreams all night and waking up ravenously horny. Though the decent effects despite that level being way too sedating stuck like the lower but unknown AA content strain and the orgasm did nothing on the POIS front besides making me a bit too tired after orgasming yesterday too. The new strain I have coming in is 6 percent and I'll report back on how that goes or if I feel I need to go lower to 3/4/5 percent. Though I found most strains don't seem to go below 5 percent AA content and it's pretty tough to find anything below 4 percent.

Anyone have anything from their own experiences that might be useful in putting together what is going on here?

edit - Now to me it's clear POIS is a symptom of a much bigger cluster of dysfunction going on in my body that also as in recent years led something going wrong in an entirely different direction where I lost everything including POIS but regained it after almost like a seesaw that due to some genetic defects causes my production/conversion of many hormones/neurotransmitters to be completely unstable and getting stuck in loops. I remember from my story on the whole PFS/PSSD saga that I relentlessly attempted to resolve after I started getting dietary flucuations in my state and managing to integrate D3 again which was an adventure on it's involving rebounding off of calcifediol and then getting on the rice flour only additive vegan version at the dose I take today of 2500 iu that 2 types of fish when cooked gave me different positive modulations in my condition. Cooked salmon and tuna. Tuna was more energizing but cooked salmon rebounding eventually led to a great deal of progress after some weeks of having it in my dinner often. The other night I got some more energy from cooked tuna and today seeing that I'm a bit more sedated than usual due to having the hops with the too high alpha acid content and being tired from having another orgasm today with no traditional POIS symptoms besides a slight touch of an immune flare I'm going to try the tuna again tonight and see what happens. I'm also not going to have the hops tonight because I want to clear my system again from high AA hops residue first and then the next night I'll get back on the lower AA (yet unknown amount) hops from Biokoma. I also have a 6 percent strain and a 3.8 percent strain on the way to feel my way around what might be going on here. Will post updates when I have them.

Lifestyle Diary and POIS Summaries / Re: Warrior's Health Journal & POIS Protocol
« Last post by Warrior on April 18, 2025, 02:31:56 AM »
Claude AIs interpretation of OAT, & suggested supplements. Much of this converges with ChatGPTs interpretation & suggestions. I intentionally took OAT (organic acids test) during peak POIS & food sensitivity symptoms.

Supplement Recommendations Based on Organic Acids Test Results

Primary Concerns

Fungal/Yeast Overgrowth

Multiple elevated fungal markers including citramalic acid (61), 3-oxoglutaric acid (66), furancarbonylglycine (7.9), and tartaric acid (61)
High glycolic acid (87) without proportional oxalic acid elevation suggests fungal metabolism

Mitochondrial Dysfunction

Elevated succinic acid (8.7) indicates respiratory complex 2 dysfunction
Elevated 3-methylglutaric acid (0.73) suggests impaired leucine metabolism

Neurotransmitter Imbalance

Elevated HVA/VMA ratio (1.8) indicates decreased conversion of dopamine to norepinephrine

Detoxification Issues

Elevated 2-hydroxyhippuric acid (1.1) indicates salicylate exposure or metabolism issues

Recommended Supplement Regimen
1. Anti-Fungal Support

Caprylic Acid: 1000mg twice daily
Justification: Natural medium-chain fatty acid with antifungal properties effective against Candida and Aspergillus species identified in the test results
Berberine: 500mg three times daily
Justification: Botanical compound with broad-spectrum antimicrobial activity, particularly effective against fungal overgrowth
High-Potency Multi-Strain Probiotic: 50+ billion CFU daily
Justification: Helps restore microbial balance to counter fungal overgrowth; specifically mentioned in the test interpretation

2. Mitochondrial Support

Coenzyme Q10: 200mg daily
Justification: Essential for mitochondrial electron transport chain function; directly addresses the elevated succinic acid (respiratory complex 2 marker)
L-Carnitine: 1000mg twice daily
Justification: Supports fatty acid transport into mitochondria; recommended in test interpretation for 3-methylglutaric acid elevation
Riboflavin (B2): 100mg daily
Justification: Cofactor for mitochondrial function; specifically recommended for methylglutaric acid elevation in the test interpretation
Nicotinamide (B3): 500mg daily
Justification: Precursor to NAD+, essential for mitochondrial energy production; specifically recommended in test interpretation

3. Neurotransmitter Support

Vitamin C: 1000mg twice daily
Justification: 1) Patient's level is below mean (12); 2) Required cofactor for dopamine ?-hydroxylase (addresses elevated HVA/VMA ratio)
Copper Glycinate: 2mg daily
Justification: Essential cofactor for dopamine ?-hydroxylase enzyme; addresses elevated HVA/VMA ratio

4. Detoxification Support

N-Acetyl Cysteine (NAC): 600mg twice daily
Justification: Precursor to glutathione; supports detoxification pathways and helps address the 2-hydroxyhippuric acid elevation
Calcium-D-Glucarate: 500mg twice daily
Justification: Supports phase II detoxification and glucuronidation pathways for clearance of toxins

5. Vitamin Optimization

Pyridoxal-5-Phosphate (B6): 50mg daily
Justification: Low pyridoxic acid (0) indicates B6 deficiency; active form is more bioavailable
Pantothenic Acid (B5): 500mg daily
Justification: Level is below optimal (2.2 vs reference mean)
Calcium Citrate: 250mg with meals
Justification: Helps bind oxalates in the GI tract to reduce absorption; beneficial with elevated oxalic acid (88)

6. Additional Support

Magnesium Glycinate: 300mg daily
Justification: Supports over 300 enzymatic reactions including mitochondrial energy production; helps address muscle symptoms from oxalate issues
Vitamin E (mixed tocopherols): 400 IU daily
Justification: Antioxidant support recommended in test interpretation for oxalate issues and mitochondrial dysfunction

Implementation Recommendations

Staged Introduction: Introduce supplements gradually over 2 weeks to monitor tolerance
Priority Sequence:

Begin with antifungal protocol (probiotics first, then gradually add caprylic acid and berberine)
Next add mitochondrial support (CoQ10, L-carnitine)
Follow with vitamin support and detoxification support

Diet Modifications:

Consider low oxalate diet (avoid spinach, soy, nuts, berries)
Eliminate potential exposure to synthetic salicylates (aspartame)
Consider food sensitivity testing due to bacterial markers

Follow-up Testing:

Repeat Organic Acids Test in 3 months to evaluate efficacy
Consider GPL-TOX testing as recommended in interpretation to assess toxic chemical exposure contributing to mitochondrial dysfunction
